• The Brokers Agreement, the Agreement with Correspondence. Make sure they are completely filled out, signed (in 2 places) and returned to our office.
• A copy of your D.O.I. License
• A copy of your E. & O. Declaration Page
• A copy of your current Agency Bond

Please note, before we will be able to issue a binder on any account, we require:

• Request to Bind in Writing
• A deposit of 25% to 50% of the premium plus all fees.
• When it is applicable a signed D-1 form
• A Terrorism form signed and marked accepted or rejected
• SL-2 form - completed and signed
